Animal Kingdom

Elephant

The animal kingdom is coming to Lovely Lane's summer daycare!

This summer at Lovely Lane Summer Daycare, we will investigate and study the world of animals. Where do they live? What do they eat? What are their babies called? How do they interact with each other? We will explore the animal kingdom with projects, arts, crafts, food, games, field trips, speakers, presenters, and more!

Our summer program runs from June 4 through August 17 from 6:30 a.m. to 5:45 p.m. and is open to students entering Kindergarten through sixth grade. Tuition is $155 weekly plus a $25 monthly activity fee. There is a $35 per day drop-in fee if this will work better your family. We are also offering a 30% discount for siblings this summer! Each child who attends will need their own pool pass and a Lovely Lane t-shirt for field trips.

Summer DHS Forms

Registration for our summer program, Animal Kingdom, has begun! In addition to the Registration form and $35 non-refundable check, you also need to fill out the required DHS forms. These forms are below and are Microsoft Word forms so you can fill them out on the computer, print them off, and then sign them and bring them in. If you prefer to fill out the paper copies, they can be picked up from Mrs. Barnes' office.

In addition to the registration form, the required forms are:

Health Assessment

Pick-up Authorization

Travel Authorization

Enrollment Agreement

Intake Sheet

Parental Emergency Medical Consent

Sunscreen Permission

Parent Handbook Agreement (to be filled out after reading the Parent Handbook)

Emergency Card

Immunization Form must be printed off and brought in from your doctor's office as well.
